PLEASE NOTE: Development of the template forms is not included in the module price listed in your contract. Each form has a base price that can be provided by request, and any changes outside of basic branding of the form will incur additional costs. Your Technical Consultant will provide a quote that includes any changes you have requested, which will need to be approved prior to the forms being built. Once the forms have been built, the Development fees will be invoiced through our regular maintenance process.
