Apply for an ATM or Visa Debit Card

In order to apply for a card for your account, please complete the form below. If your card has been lost or stolen, or if you have any questions about completing the application, please call the credit union at (414) 665-3423.




I understand that if I, or another account owner, give a card and/or the confidential PIN to another person, he or she will be able to use the card to access my accounts in the same manner that I can whether or not that person has signed the signature card. I agree to notify the credit union promptly of any known or suspected loss, theft, disappearance, or unauthorized use of any card, code, check or passbook permitting access to this account.

Notice: If you are a party to a joint account, you appoint each party as your attorney with power to appoint one or more agents with power to use the card to make withdrawals from such account, and each is individually and jointly responsible for any obligations incurred from use of the card, including use by any other person authorized by any of you.

I/We certify that this application is complete and true. I/We agree that retention of use of the card constitutes agreement to all of the terms and conditions set forth in the ATM/Debit Card Agreement, Electronic Funds Transfer Disclosure, and Fee Schedule, and to any changes made therein from time to time. I/We authorize Northwestern Mutual Credit Union to make whatever credit inquiries are deemed necessary in connection with this application.
