As part of its program to provide a safe and secure work environment, Foundation One Bank ("Bank") will conduct background investigations on all applicants, and for certain positions the background investigation will include a credit check after you have received a conditional offer of employment. The results will be considered in any employment decision and will result in a rejection of an applicant for employment, or the termination of any employee who is permitted to start work prior to the time the results of any background investigation are received by the Bank. The Bank expects you to sign this consent form if you wish to have your application for employment considered. If you refuse to sign the consent form you will be denied employment.


DISCLOSURE AND CONSENT CONCERNING CONSUMER REPORTS FOR EMPLOYMENT APPLICANTS AND EMPLOYMENT PURPOSES.
