APPLICANT STATEMENT
I certify that all information I have provided in order to apply for and secure work with Community Bank and Trust is true, complete and accurate.
I understand that any information provided by me that is found to be false, incomplete or misrepresented in any respect, will be sufficient cause to (1) cancel further consideration of this application, (2) immediately discharge me from Community Bank and Trust, whenever it is discovered.
I hereby authorize, without reservation, Community Bank and Trust, its representatives, employees or agents, to contact and obtain information from all references (personal and professional), employers, public agencies, licensing authorities and educational institutions and to otherwise verify the accuracy of all information provided by me in this application, resume or job interview. I hereby waive any and all rights and claims I may have regarding the employer, its agents, employees or representatives, for seeking, gathering and using such information in the employment process and all other persons, corporations or organizations for furnishing such information about me.
I understand that Community Bank and Trust does not unlawfully discriminate in employment and no questions on this application are used for the purpose of limiting or excusing any applicant from consideration for employment on the basis prohibited by applicable local, state or federal law.
I understand that this application remains active for 90 days. At the conclusion of that time, if I have not heard from Community Bank and Trust and still wish to be considered for employment, it will be necessary to reapply and complete a new application.
If I am hired, I understand that I am free to resign at any time, with or without cause, and without prior notice, and Community Bank and Trust reserves the same right to terminate my employment at any time, with or without cause, and without prior notice, except as may be required by law. This application does not constitute an agreement or contract for employment for any specified period or definite duration. I understand that no supervisor or representative of Community Bank and Trust is authorized to make any assurance to the contrary and that no implied oral or written agreements contrary to the foregoing express language are valid.
I also understand that if I am hired, I will be required to provide proof of identity and legal authority to work in the United States and that federal immigration laws require me to complete an I-9 Form in this regard.
I understand that Community Bank and Trust utilizes pre-employment drug screening and that a positive result will disqualify me from employment.

If submitting your Application for Employment electronically by email, this document informs you of
your rights regarding the receipt of legally-required disclosures, notices, consents and information associated with
your application to Community Bank and Trust.
The hardware and software requirements needed to access
and retain the electronic records
include: Internet Explorer 6.0 or above, Firefox 2.0 or above, Microsoft Word and hardware capable of
running this software.
In order to submit your Application for Employment with Community Bank and Trust
electronically via email, we need your consent. By completing and emailing your application to
Community Bank and Trust you:
- Acknowledge receipt of this document and consent to the electronic delivery of such disclosures.
- Consent to receive communications and disclosures related to the processing of your Application for Employment with Community Bank and Trust in electronic form. This consent includes any disclosures required by federal, state or local law, including, but not limited to, disclosures under the federal Fair Credit Reporting Act.
- Can access the electronic disclosures in the designated formats described herein.
- Consent to having all disclosures from Community Bank and Trust provided or made available to you in electronic form.
After submitting this Application for Employment and accompanying Release Authorizations electronically via email, you may withdraw your consent by informing
Community Bank and Trust in writing to: Community Bank and Trust, Attention Human Resources, 100 S. Wood Street, Neosho, Mo 64850. If you decide to withdraw your consent, the
legal validity and enforceability of prior electronic disclosures will not be affected, and you will no longer be
considered for employment.
NOTIFICATION AND AUTHORIZATION FORM FOR
EMPLOYMENT CREDIT REPORT
In connection with my application for employment, or continued employment, I authorize any credit bureau, employer, former employer, educational institution and other persons contacted to release information regarding my records, investigative consumer report, character, performance, qualifications, background and reasons for termination of past employment to Community Bank and Trust or its agents from any responsibility or liability, and also any injuries or damages not now known or which may later develop, resulting from the disclosure of such information. I realize and agree that this information may be obtained prior to my employment or during my employment with Community Bank and Trust. I also realize that this information may be obtained for internal security reasons and hereby consent to this policy as a term and condition of employment.
I understand that the decision to hire is solely that of Community Bank and Trust, and although said decision may be based upon information gathered by the credit reporting agency, I release the credit reporting agency from any and all liability surrounding or related to the employer's decision. If an adverse employment decision is made due totally or partially to the information on the credit report, Community Bank and Trust will provide me a copy of the credit report, a summary of my rights under the Fair Credit Reporting Act and the source of credit report so that I may contact them if I wish. |
